    ikev2: in next_v2_transition() use v2_msgid_queue_initiator()
    
    i.e., really schedule the next exchange and then return
    STF_OK so that the current exchange completes.
    
    also:
    
    - tweak complete_protected_but_fatal_exchange() to find better
      transitions
    - tweak success_v2_state_transition()'s don't log OE code
