    ikev2: merge STATE_V2_IKE_AUTH_CHILD_I0 into NEW_CHILD_I0
    
    Also clean up cosmetic child sa transitions.  The real
    transitions are attached to the IKE SA.

    state: add CAT_OPEN_CHILD_SA, use for NEW_CHILD_I1 et.al.
    
    and not ESTABLISHED_IKE_SA!
    Review other state categories.
    Move state_category_names to libswan so it is checked.
