[Swan-dev] state m/c #0of3 cleanups

 I've a branch with a number of somewhat related changes, as a heads
up, I'm going to post comments on each of the changes here.

The end result is two fold:
- pluto is more likely to handle re-transmits during init (it can
currently drop them)
- the state machine driver is, I would argue, more transparent

(the other change is some cosmetic the above lean on)
