    ikev2: pass MD=null into IKE_INTERMEDIATE, and IKE_AUTH initiators
    
    Like for CREATE_CHILD_SA and IKE_SA_INIT initiators.
    
    Anything the IKE_INTERMEDIATE and IKE_AUTH initiators need from
    the IKE_SA_INIT response must have been saved in struct ike_sa.
    (any IKE_AUTH initiator code trying to get an IKE_SA_INIT response
    value from the previous message was broken, and has(?) been fixed).
