    ip: update ip_protocol
    
    - drop <<typedef struct ip_protocol ip_protocol>>
      unlike the other ip_* types, this isn't ment to be opaque.
      <<typedef const struct ip_protocol *ip_protocol>> might be useful
    - rename conversions to protocol_from*() (everything else is *_from_*())
