    ikev2: when initiator rejects first child from responder, initiate delete
    
    Because the state transition is tied to the IKE SA, and has no
    attached whack (it was detached when IKE established), the whack
    messages seem contradictory:
    
      #2: initiating delete
      #2: deleting state (STATE_V2_IKE_AUTH_CHILD_I0) and NOT sending notification
    
    read the log file and all is revealed.

    ikev2: pass the (possibly NULL) child to v2_msgid_queue_initiator()
    
    ... and assume, when ST parameter is NULL, the IKE sa is the
    exchange owner.
