    x509: replace load_end_cert_and_preload_secret() with add_end_cert_and_preload_secret()
    
    Move code figuring out cert source to extract_end().
    Simplify nss_reread_cert() - no need to work around code figuring out ...
    BTW, reread doesn't handle raw keys.
