    packets: consolidate *out_struct() functions
    
    - in pbs_out_struct() use log_pbs_out() (drop logger parameter)
    - change out_struct to a macro wrapper (it lacks a size parameter)
    - eliminate open_output_struct_pbs()
    - in close_output_pbs() zap the .out_logger
