    logging: cleanup complete_v2_state_transition()'s fail path
    
    - replace the log lines like:
        <state-name>: NOTHING_WRONG
      with:
        state transition '<transition-story>' failed"
      like the comment says "clearly this log line is silly and needs
      fixing".
    
    - add magic RC_NOTIFICATION+reason prefix to log message that a reply
      is being sent sending reply
