    packets: replace MD's .isa_[ir]cookie with .isa_ike_{initiator,responder}_spi
    
    Since both state and msg_digest use ike_sa_t, all the operations
    like mem*(..., COOKIE_SIZE) can be eliminated.
    
    Avoid immediate code churn by #defining isa_[ir]cookie.
