    ikev2: set a fake MD's .from_state to .st_state and not STATE_IKEv2_BASE
    
    MD's .from_state == STATE_IKEv2_BASE was being used as a magic marker
    to indicate an initiator's first request.
    
    In success_v2_state_transition(), near "Adjust NAT but not for initial
    state", use (arguably slightly better) explicit checks for all
    possible initiator initial states.  Suspect code can be deleted.
