    ikev2: start the IKE_SA_INIT responder in STATE_PARENT_R0
    
    Had ST in STATE_PARENT_R1, MD in STATE_IKEv2_BASE, and the transition
    in STATE_PARENT_R0.
    
    Update 'don't update nat' hack.
