    ikev2: eliminate send_v2_notification() used by send_v2_notification_using_{md,st}()
    
    For send_v2_notification_using_md(), the packet is directly sent
    using knowledge from the MD (rather than creating fake_st and using
    code intended for sending out a packet using state information).
