    include: add __func__ and fmt,.. args to libreswan_passert_fail()
    
    Define PASSERT_FAIL so its use is easier, update bad_case() et.al.
    to use PASSERT_FAIL.
    
    Failure output changed to:
       ASSERTION FAILED: <reason> (in <func> at <file>:<line>)
    to reduce the odds that <reason> is lost in truncated output.
