    building: re-implement manpages.mk; always build manpages
    
    New make variable MANPAGES replaces EXTRA[1-8]MAN et.al.
    The targets "programs install-programs" do not build man pages
    but "all" and "install" do.
    Both pluto.8 and ipsec_pluto.8 installed!
    All generated man pages removed.
    All .xml files renamed to match the man-page they generate.
