    POLICY_ISAKMP was really a method of indexing tables of sadbs.
    Rename as "sadb_index" and move definition to spdb.h
    Change code to use result directly as a subscript,
    avoiding the need for "policy_index" variables.

    Simplify some code involved with closing whack socket.
    Fixes leak introduced in 3eda2666ddb763ee3d659e56ec862bfe93d16cef
    which in turn was a fix for a sometimes incorrect closing of a whack socket.
